When does long term care insurance start paying?

Right now, I am comparing ways to insure against future home care, assisted living, or nursing facility costs. Right now, policy language includes benefit triggers, waiting periods, daily benefits, and possible premium increases. The agents I have spoken with do not describe the policy the same way, and I need to distinguish what is guaranteed in the contract from what is only projected.
