What is a long term care plan besides insurance?

The situation is that I have watched older relatives need help with bathing, dressing, transportation, and eventually facility care. The situation is that the costs were much higher than our family expected, and I do not want my own plan to assume Medicare or my children will cover everything. What I am trying to do is fit this into a broader financial plan rather than treat it as an isolated product or account question.
