Does Medicare pay for long term nursing home care?

I have seen relatives need increasing help with daily activities and eventually move into a care facility, and the cost was far higher than our family expected. I am trying to build my own plan without assuming that Medicare will pay for an extended nursing home stay. What does Medicare generally cover in this situation?
