Can naming beneficiaries avoid probate?

Several of my accounts allow me to name a beneficiary directly. If I add beneficiaries to everything possible, does that usually keep those assets out of probate? I also want to understand what happens if a beneficiary dies first or the form is never updated after a divorce or other family change.
